2. Service Agreement

Peaceful Pathways Home Healthcare, LLC
This Service Agreement (“Agreement”) outlines the terms for home healthcare services provided to the client (“Client”) by Peaceful Pathways Home Healthcare, LLC (“Company”).

1. Services Provided

The Company will provide home healthcare services which may include:

• Personal care

• Companionship

• Assistance with daily living

• Medication reminders

• Light housekeeping

• Mobility support

• Additional services as agreed upon

A customized Care Plan will be created based on the Client’s needs.

2. Scheduling

Services will be delivered according to the mutually agreed schedule. The Client agrees to provide notice for schedule changes at least 24 hours in advance when possible.

3. Payment and Billing

• Invoices are due upon receipt unless otherwise specified.
